Our clinical coordination team will follow up by phone or email to gather any additional details needed, verify your insurance information, and help schedule a consultation at the earliest available time.

Your Consultation Is Scheduled
We work to schedule consultations promptly, especially for urgent referrals. You'll receive all the information you need to prepare for your visit, including any records or imaging we may need in advance.

Information to Have Ready

Having the following information available when you contact us helps us serve you more efficiently and reduces delays in scheduling your consultation:

Personal Information

  • Full legal name and date of birth
  • Current phone number and email
  • Insurance provider and member ID
  • Referring physician name (if applicable)

Medical Records

  • Relevant imaging (MRI, CT, angiogram) on CD or via secure portal
  • Imaging reports from your radiologist
  • List of current medications
  • Brief summary of your condition or symptoms
Availability

Office Hours

Our office is open Monday through Friday, 8:00am to 5:00pm. Voicemails and form submissions are reviewed by our clinical coordination team.
